Get Organized.


If you are caring for an elderly loved one you will need to keep their health records straight and organized. A personal health record is a complete and accurate summary of the health and medical history of an individual. A PHR can be kept in a paper format, on a computer software program, online, or on a disc. A PHR allows the individual to keep his/her information updated and organized on a regular basis and helps to communicate pertinent information to medical professionals.

Take the time to create a grab’n’go portfolio that has copies of your loved one’s health insurance benefits, medical history, list of medications and any other pertinent information that would be needed in a medical emergency. This could make a spur-of-the moment hospital visit go much smoother and more efficiently.

Create a calendar for the year, post it in your loved one’s kitchen and keep a calendar for yourself. Write down all doctor’s appointments, home care visits schedule, and any other scheduling information to help keep you and your loved one on track.
